Community Care TASMANIA is accredited under the Tasmanian government’s Home and Community Care (HACC) Programme and the Australian Government’s Individual Support Program, (ISP). These provide funding for services to support younger people (less than 65 years) and their carers.
These are designed to assist those whose capacity for independent living is at risk through mental or physical disability or accident and/or who are at risk of premature or inappropriate admission to long term residential care.
Community Care TASMANIA is currently supporting a small number of younger clients funded under these programs.
In some cases, younger people with disabilities, dementia or special care needs may be able to access a Home Care Package – if the person has been assessed and approved by an Aged Care Assessment Team (ACAT), and a home care provider is able to offer an appropriate package for the person.
